How To Apply For CE
 
The Ohio State Board of Optometry has approved two hours of continuing
education credit for the online self-tutorial courses. There is a $30
processing fee per course required to receive credit.
 
To apply for CE credit, please complete the following guidelines:
 

1) Print out the Test Questions
2) Be sure to clearly PRINT your name, address and telephone number in the space provided on the test answer sheet
3) Complete the test
4) Mail completed test answer sheet and a check for $30 payable to The Ohio State University to:
