Cancellations of registration must be in writing. Any course is subject to cancellation by the EastWest Eye Conference.
The EastWest Eye Conference claims no liability for the acts of any supplier or speaker at this meeting or for the safety of any attendee while in transit to, from, or during this meeting. Attendees who purchase non-refundable airline tickets do so at their own risk. The EastWest Eye Conference reserves the right to cancel this function without penalty. The total amount of liability to the Conference will be limited to a refund of fees paid by the attendee.
Workshops provide an excellent opportunity for hands-on learning and in-depth tutorials on the latest techniques, products and technology in optometry. However, class size is limited. Therefore, registration preference is given to optometrists first. If there is space available at the time of the workshop, students may be admitted.
Physician Payment Sunshine Act:
The Open Payment/Physician Sunshine Act, 6002 of the Affordable Care Act (P.L. 111-148)(2010) requires drug and device manufacturers to report attendance registration information for an EastWest Eye Conference 2016 continuing education courses and/or meals or other items of value which are supported (in whole or in part) by grant funding from such companies. The OOA will help sponsors comply with the Sunshine Act by collecting information they need to know about optometrists who attend the meeting
EastWest attendees grant permission to the Ohio Optometric Association and its agents to utilize, in perpetuity, the attendee’s image or likeness in an effort to promote Ohio Optometric Association events and meetings. Attendees waive any right to inspect or approve the finished product or products, the advertising copy or other matter that may be used in connection therewith or the use to which it may be applied.
The registration fee includes access to all Exhibit Hall activities, Breakfast Seminars, and admission to the exclusive Friday Night Party at the Rock and Roll Hall of Fame.
All courses require a course card for admission. If you decide to attend a different course than what you registered for, you may exchange cards on-site. There is no refund for unused course cards and there is no refund for on-site course cancellation. No refund for course cancellation after September 12.
Registration for spouse or guest can be done online under either the OD Registration or AEP Registration Tab. A spouse or guest can register for the Rock and Roll Hall of Fame Party ($59), the OOA Presidents Banquet ($59), or a pass for the Exhibit Hall ($25).
Replacement Course Cards/Name Badges
Replacement for a lost name badge is $5. Lost course card(s) will be reprinted at the cost of the course as stated.
